COURSE DESCRIPTION
Module 4 focuses on Evaluation and Management documentation standards based on the Medical Decision Making (MDM) level determination, as defined by the CMS 2021 guidelines applicable for Traditional Medicine. Learn how to conduct and document a health history, functional questionnaires, and relevant physical examination. Demonstrate knowledge of cultural competency documentation. Reinforce documentation of co-management and referral to other health professionals. This class will exercise critical thinking and professional judgment in the assessment and treatment planning. Students will become proficient in collaborating with patients to develop short-term, medium-term, and long-term treatment plans, modifying plans as necessary to accommodate changes in the patient’s condition, and assessing patient outcomes. Guidelines for documenting phone calls will be introduced. Templates of the associated forms and clinic policies will be reviewed.

- Learn Documentation Guidelines for Establishing the Differential Diagnosis; Assessing for Red Flags and Adverse Events; Recording Referrals: and Developing a Management Plan
- Learn Documentation Guidelines for Phone Call Communication
